Are you interested in becoming a new patient? We would like to make your first visit as easy as possible. Click "Schedule Now" below and look for the correct forms to fill out in your Charm patient health portal. Please complete your new patient intake form and submit before your first appointment. If you're scheduling on-line, you will be asked to enter your credit card number, though no charge will occur until after your visit.
Visit Fees
In order to better serve you as a patient, we do not bill insurance directly or accept insurance payments for our services. Insurance companies cast many regulations as to what we can cover in a visit and be paid for. In order to provide the most comprehensive care, we require payment at time of service.
After each visit we will provide you with an electronic superbill that you can submit directly to your insurance company for reimbursement based on what your out of network coverage is.
TIME OF SERVICE RATES:
New patient visits, 90 minutes, $395
Follow-up visits, 60 minutes, $235
Follow-up visits, 45 minutes, $180
Follow-up visits, 30 minutes, $130
HOLISTIC PELVIC CARE:
New patient visits, 90 minutes, $390
Follow-up visits, 60 minutes, $270
